Continuous Improvement Project Intake Form

The Project Intake Form is an important part of our process. It captures the most relevant information about your continuous improvement project and gives us a better understanding of the scope of your project and the support you would require from us.

Business Intelligence Reporting Intake Form

One of the services we offer is data tracking and reporting. The intake form allows our Business Intelligence team to gather context in just a few simple questions and gain a clear understanding of how they can help.

Project Prioritization and Change Management Matrices

These templates should be used to assess and prioritize the delivery of up to three different projects.

Project Charter – PowerPoint Template

A one-page template to be updated by the project manager, using information captured in the Project Intake Form. The Project Charter will be used to present a new project summary information to the Governance Committee for evaluation.

Note: The Project Charter is also included as part of the project kick-off template below.

Project Governance & Team Structure – PowerPoint Template 

The standard Operational Excellence PowerPoint template to be used by project managers to show the members of a project’s steering group, project team, and reference group.

High-Level Project Plan – PowerPoint Template

Use this high-level project plan to provide an overview of timelines to your governance group, and other key stakeholders. For a more detailed version, use the Project Plan – Excel Template below, or for more sophisticated project plans, MS Projects.

Detailed Project Plan – Excel Template

The project plan uses Gantt charts to schedule and monitor specific tasks and resources over a project’s lifespan. The visualization allows employees to get ahead of deadlines, track progress, and identify project milestones. This project plan version in excel is an alternative for users who are not familiar with MS Projects. For more sophisticated project plans, we recommend that you use MS Projects instead. Please contact UBC IT if you’d like to have MS Projects installed on your computer.

Operational Excellence Project Risks Matrix

This template should be used to highlight the top 3~5 project risks. There are two versions available in PowerPoint and Excel formats.

Project Action and Issue Log – Excel Template

Project managers can use this template to facilitate the follow-up and tracking of all project-related actions and issues by priority and status.

Business Process Review Action and Issue Log – Excel Template

Use this template to facilitate the follow-up and tracking of a business process review related actions and issues by priority and status.

Business Case – Excel Template

This is the standard Operational Excellence business case template for project managers to evaluate potential net financial savings and payback in years for continuous improvement initiatives.

Operational Excellence Project Kick-Off PowerPoint Template

This is the standard Operational Excellence PowerPoint template designed for project managers to kick off a new project and align all Steering Committee and project team members towards the same goals and expectations.

Change management consists of strategies designed to support people as they transition through change. It is important to determine the size and level of change before delivering a project, as it helps determine the most effective change tools and techniques.
